This form is to apply for Cash Units in the Balmain Discrete Mortgage Income Trusts (Trust). The minimum initial investment in Cash Units is AUD $50,000.

By commencing this form, you confirm that you have the express consent of all applicants and account holders to complete the application on their behalf (including consenting to Balmain Private accessing and confirming information on all individual investors with a credit reporting agency for the purpose of AML/CTF Legislation).

Further, you confirm that all account holders are aware that you may be provided with an online login and full transactional access over the account, dependant on the information provided to Balmain, including the ability to redeem funds to the nominated bank account provided on this form. Please note that the Primary Applicant is legally required to ‘sign’ the form electronically. The Primary Applicant must be one of the account holders, and the email address provided for the Primary Applicant must belong to that individual (as they will be sent a 4 digit ‘Completion Pin’ to their nominated email address to complete the form).
